2009 1KR Yaris engine shuddering in 1st gear

Hello people, I have seen similar questions asked here but mine is a bit different. My Yaris is a 2009 1KR-FE with the 5spd manual transmission. It has about 110,000KMs on the odo.

Since a while back I have noticed that the car shudders quite violently when trying to take off from stand still. I am quite sure it's not related to releasing the clutch early or not giving more gas, because it still does it when the RPM is well above 2000. Even more puzzling is, the car is normal when its cold. In the morning or when I have parked it for some time, it doesn't do the weird shuddering. It starts to shudder when its all warmed up and becomes more violent when I spend time in traffic.

Any ideas why this is happening?

Sounds like burned clutch plate ...
Do you stand on clutch pedal at traffic jam or select neutral when possible ??

I always put it in Neutral.

I don't know if it's related, but the transmission always grinds when I put it into reverse.

Reverse gear is not synchronized !
Could also be tacking clutch release bearing ...

How serious is the problem? Should I take an immediate action?

Can't say yes or no ..
It's quite annoying feature to be honest !
I would change clutch if my car have this kind feature !
