Header w/o O2 sensor hole....??

Likely its an xB header, it does not have a bung. And if in fact it is an xB header the angle of the flange will be incorrect as well, it wont bolt up with a Yaris down-pipe. Send it back and get the right part.

It's OBX. Sold as direct fit for Yaris. I would have expected something like this from one of the no-name, Megan knockoffs off eBay... Well, guess I need to start calling some shops to price it out for the custom addition.

I would double check the part number, the only way the O2 would be missing from it is if they sent you the xB unit. If you have to have an O2 bung added, and modify the collector to mate up to the Yaris down-pipe, you are going to spend more than buying the right piece.
