I don't think that I ever drive fast enough to use the 6th gear on a manual. I tend to stay in the 65-70 mph range maximum (which is still in the 2800 RPM range in 5th). All the manuals I have ever driven have been 5-speed, so the odd position of the reverse and such would likely take some getting used to - and be downright annoying for a while. It just seems like overkill to me, especially in the USA. Personally, I feel that 5-speed manuals and automatics are the "sweet spot", in terms of functionality plus overall reliability.
